This form shall be used to create a hand made crocheted item and settle in contractual form an agreement between the Crafter (Teresa Massie) and Designer/Buyer _______________________________ for item(s) described below. This notice is to be signed by both parties and work will start once Materials cost and 50% of Labor cost is estimated and collected by the Crafter. All extra or unforeseen expenses will be discussed by Crafter and Buyer before any extra changes/purchases are made and will be noted on this contract. Pease note, however, that this may alter the agreed upon date of completion, which will also be noted on this contract.

Once the materials have been purchased, all sales are final. No refunds, returns, or exchanges will be allowed. Refusal to purchase final product will result in forfeit of any monies paid upfront to start project. Crafter may reject a prospective buyer’s design or design change for any reason. Signatures on this contract signifies an understanding to the stipulations. Crafter also agrees to have the item finished by an agreed upon date (______________________). If item is not completed by the agreed upon date, Buyer has the right to reject finished product.
